The pelvic floor is a muscle, and it responds to training
The evidence is strong, and the timescale is longer than people expect.
Pelvic floor muscle training has some of the best evidence of any intervention in this section. A Cochrane review of 31 trials found that women doing it were around eight times more likely to report their incontinence cured, and trials in men after prostate surgery show similar direction.
The sexual effects are less dramatic but consistently positive in both sexes: improved sensation, stronger orgasm, and in men a measurable effect on erectile function and premature ejaculation. The catch is time. Studies run twelve weeks or more, and effects before eight weeks are rarely detectable.
